Skilled Nursing Visits (RN/LPN)

Scheduled visits from a licensed registered or vocational nurse for assessments, treatments, and physician-ordered care. Each visit is charted, and changes in condition are escalated to the ordering physician the same day.

  • Head-to-toe nursing assessments
  • Vitals, labs coordination & reporting
  • Injections & ordered treatments
  • Physician communication & orders

Post-Surgical & Wound Care

Structured recovery support after orthopedic, cardiac, and general surgery. Nurses manage incisions, drains, and dressings while watching for the early warning signs that send patients back to the hospital.

  • Incision & dressing management
  • Drain and wound-vac care
  • Infection surveillance
  • Pain-plan adherence support

Medication Management & Reconciliation

Polypharmacy is the most common driver of avoidable readmissions we see. A nurse reconciles every prescription, over-the-counter, and supplement against current orders — then keeps the regimen organized week to week.

  • Full medication reconciliation
  • Pill organizer setup & refills
  • Interaction & duplication review
  • Administration & adherence tracking

IV Therapy & Infusion Support

Hospital-grade infusion care without the hospital. Our infusion-experienced RNs administer ordered IV antibiotics, hydration, and specialty medications with strict line-care protocol.

  • IV antibiotics & hydration
  • PICC and midline care
  • Port access & maintenance
  • Infusion reaction monitoring

Chronic Condition Care

Diabetes, congestive heart failure, and COPD demand daily measurement, not occasional check-ins. We build a tracking routine around each diagnosis and escalate trends before they become emergencies.

  • Glucose, weight & symptom tracking
  • CHF fluid & sodium monitoring
  • COPD oxygen & breathing protocols
  • Early physician escalation

Post-Hospital Transition Care

The discharge summary is a starting point, not a plan. Within 48 hours of discharge we reconcile orders, confirm follow-up appointments, and stabilize the home routine during the highest-risk window.

  • 48-hour post-discharge visit
  • Discharge order reconciliation
  • Follow-up appointment coordination
  • Readmission risk watch

Elder & Companion Care

Aging in place works when daily life is supported, not supervised from a distance. Caregivers assist with the routines that keep clients safe, nourished, and engaged — under nurse oversight.

  • Bathing, grooming & dressing
  • Meal preparation & hydration
  • Mobility & transfer assistance
  • Companionship & engagement

Dementia & Memory Support

Memory care at home succeeds on consistency: the same caregiver, the same routine, the same calm cues. We structure the environment and the day to reduce agitation and wandering risk.

  • Consistent assigned caregivers
  • Routine & environment structuring
  • Wandering & fall prevention
  • Family coaching & updates

Palliative & Hospice Support

Comfort-focused nursing alongside your hospice or palliative team. We manage symptoms, positioning, and medications so the client's final chapter is spent at home, with dignity.

  • Symptom & comfort management
  • Hospice agency coordination
  • Positioning & skin integrity care
  • Family presence & guidance

Respite Care for Family Caregivers

Family caregivers burn out quietly. Scheduled respite gives you dependable coverage — a few hours a week or full days — with a clinician who already knows the care plan.

  • Scheduled weekly relief blocks
  • Overnight & weekend coverage
  • Travel & vacation coverage
  • Same-nurse continuity

Questions

Frequently asked questions

We work with long-term-care insurance policies and private-pay arrangements, and we help families verify benefits before care begins. Traditional health insurance and Medicare coverage for non-skilled home care is limited; during your free consultation we review your specific policy and outline exactly what is and is not covered before any commitment.

Skilled nursing visits are scheduled per visit with no hourly minimum. Daily-living and companion care engagements typically begin at four hours per visit so caregivers can complete routines safely and unhurried. Your care coordinator will recommend the smallest schedule that genuinely meets the clinical need.

Every clinician holds an active Texas RN or LPN license verified through the Texas Board of Nursing, passes a multi-state background check, completes reference verification, and enters our ongoing competency review program. Licenses are re-verified on a recurring cycle, not just at hire.

Yes — care plans are living documents. The supervising nurse formally reviews each plan every 30 days, and any clinician can trigger an earlier review when condition changes. Visit frequency, services, and nurse level can scale up or down with physician input and family agreement.

A licensed nurse staffs our on-call line 24 hours a day, 7 days a week — including holidays. On-call nurses can triage by phone, adjust the next visit, or dispatch an urgent visit when clinically appropriate. You will never reach an answering service.

After the in-home assessment, the Director of Nursing selects a primary nurse based on licensure level, specialty experience (wound care, dementia, infusion, and so on), schedule fit, and temperament. You meet the nurse before the first solo visit, and we keep that assignment consistent — continuity is a clinical decision, not a convenience.
